West Nile Virus

West Nile virus (WNV) is a potentially serious illness. Experts believe WNV is established as a seasonal epidemic in North America that flares up in the summer and continues into the fall. This fact sheet contains important information that can help you recognize and prevent West Nile virus.

What Can I Do to Prevent WNV?
The easiest and best way to avoid WNV is to prevent mosquito bites.

  • When you are outdoors, use insect repellent containing an EPA-registered active ingredient. Follow the directions on the package.
  • Many mosquitoes are most active at dusk and dawn. Be sure to use insect repellent and wear long sleeves and pants at these times or consider staying indoors during these hours.
  • Make sure you have good screens on your windows and doors to keep mosquitoes out.
  • Get rid of mosquito breeding sites by emptying standing water from flower pots, buckets and barrels. Change the water in pet dishes and replace the water in bird baths weekly. Drill holes in tire swings so water drains out. Keep children’s wading pools empty and on their sides when they aren’t being used.

What Are the Symptoms of WNV?

  • Serious Symptoms in a Few People. About one in 150 people infected with WNV will develop severe illness. The severe symptoms can include high fever, headache, neck stiffness, stupor, disorientation, coma, tremors, convulsions, muscle weakness, vision loss, numbness and paralysis. These symptoms may last several weeks, and neurological effects may be permanent.
  • Milder Symptoms in Some People. Up to 20 percent of the people who become infected have symptoms such as fever, headache, and body aches, nausea, vomiting, and sometimes swollen lymph glands or a skin rash on the chest, stomach and back. Symptoms can last for as short as a few days, though even healthy people have become sick for several weeks.
  • No Symptoms in Most People. Approximately 80 percent of people (about 4 out of 5) who are infected with WNV will not show any symptoms at all.

How Does West Nile Virus Spread?

  • Infected Mosquitoes. Most often, WNV is spread by the bite of an infected mosquito. Mosquitoes become infected when they feed on infected birds. Infected mosquitoes can then spread WNV to humans and other animals when they bite.
  • Transfusions, Transplants, and Mother-to-Child. In a very small number of cases, WNV also has been spread through blood transfusions, organ transplants, breastfeeding and even during pregnancy from mother to baby.
  • Not through touching. WNV is not spread through casual contact such as touching or kissing a person with the virus.

How Soon Do Infected People Get Sick?

People typically develop symptoms between 3 and 14 days after they are bitten by the infected mosquito.

How Is WNV Infection Treated?

There is no specific treatment for WNV infection. In cases with milder symptoms, people experience symptoms such as fever and aches that pass on their own, although even healthy people have become sick for several weeks. In more severe cases, people usually need to go to the hospital where they can receive supportive treatment including intravenous fluids, help with breathing and nursing care.

What Should I Do if I Think I Have WNV?

Milder WNV illness improves on its own, and people do not necessarily need to seek medical attention for this infection though they may choose to do so. If you develop symptoms of severe WNV illness, such as unusually severe headaches or confusion, seek medical attention immediately. Severe WNV illness usually requires hospitalization. Pregnant women and nursing mothers are encouraged to talk to their doctor if they develop symptoms that could be WNV.

What Is the Risk of Getting Sick from WNV?

  • People over 50 at higher risk to get severe illness. People over the age of 50 are more likely to develop serious symptoms of WNV if they do get sick and should take special care to avoid mosquito bites.
  • Being outside means you’re at risk. The more time you’re outdoors, the more time you could be bitten by an infected mosquito. Pay attention to avoiding mosquito bites if you spend a lot of time outside, either working or playing.
  • Risk through medical procedures is very low. All donated blood is checked for WNV before being used. The risk of getting WNV through blood transfusions and organ transplants is very small, and should not prevent people who need surgery from having it. If you have concerns, talk to your doctor.

Pregnancy and nursing do not increase risk of becoming infected with WNV. The risk that WNV may present to a fetus or an infant infected through breastmilk is still being evaluated. Talk with your care provider if you have concerns.

What Is the CDC Doing About WNV?

CDC is working with state and local health departments, the Food and Drug Administration and other government agencies, as well as private industry, to prepare for and prevent new cases of WNV.

Some things CDC is doing include:

  • Coordinating a nation-wide electronic database where states share information about WNV
  • Helping states develop and carry out improved mosquito prevention and control programs
  • Developing better, faster tests to detect and diagnose WNV
  • Creating new education tools and programs for the media, the public, and health professionals
  • Opening new testing laboratories for WNV
  • Working with partners on the development of vaccine

What Else Should I Know?

If you find a dead bird: Don’t handle the body with your bare hands. Contact your local health department for instructions on reporting and disposing of the body. They may tell you to dispose of the bird after they log your report.

When humans become infected with the West Nile Virus they typically display one of the following three stages of symptoms:

  • No symptoms – Four out of five people will show no symptoms of the WNV and recover without ever knowing they were infected.
  • Mild symptoms – Similar to the flu such as fever, nausea, skin rash, headache, swollen lymph glands and muscle stiffness lasting between a few days and several weeks. People infected with WNV will begin to show symptoms between 3 and 14 days after being bitten by the infected mosquito. These mild symptoms of West Nile Virus can be overcome by most normal healthy people with over-the-counter cold treatments and antibiotic medications.
  • Extreme illness- Can occur in 1 out of 150 people, especially if the infected person is elderly (50 years or older) or has a weak immune system (suffering from HIV, AIDS or cancer). Serious symptoms of the West Nile Virus can include high fever, disorientation, coma, convulsions, vision loss, neck stiffness, numbness and paralysis. People suffering from serious WNV effects can be ill for several weeks and must be hospitalized to receive the proper treatment. If left unattended WNV can lead to permanent neurological affects, encephalitis (inflammation of the brain), meningitis (inflammation of the brain’s membrane and spinal cord), memingeoncephalitis (inflammation of the brain and membrane surrounding it) and may even result in death.

Repellents are an important tool to assist people in protecting themselves from mosquito-borne diseases.

The Center for Disease Control (CDC)  recommends the use of products containing active ingredients which have been registered by the U.S. Environmental Protection Agency (EPA) for use as repellents applied to skin and clothing. EPA registration of repellent active ingredients indicates the materials have been reviewed and approved for efficacy and human safety when applied according to the instructions on the label.

Repellents for use on skin and clothing:
CDC evaluation of information contained in peer-reviewed scientific literature and data available from EPA has identified several EPA registered products that provide repellent activity sufficient to help people avoid the bites of disease carrying mosquitoes. Products containing these active ingredients typically provide reasonably long-lasting protection:

  • DEET (Chemical Name: N,N-diethyl-m-toluamide or N,N-diethly-3-methyl-benzamide)
  • Picaridin (KBR 3023, Chemical Name: 2-(2-hydroxyethyl)-1-piperidinecarboxylic acid 1-methylpropyl ester )
  • Oil of Lemon Eucalyptus* or PMD (Chemical Name: para-Menthane-3,8-diol) the synthesized version of oil of lemon eucalyptus
  • IR3535 (Chemical Name: 3-[N-Butyl-N-acetyl]-aminopropionic acid, ethyl ester)

EPA characterizes the active ingredients DEET and Picaridin as “conventional repellents” and Oil of Lemon Eucalyptus, PMD, and IR3535 as “biopesticide repellents”, which are derived from natural materials. For more information on repellent active ingredients see Link

Published data indicate that repellent efficacy and duration of protection vary considerably among products and among mosquito species and are markedly affected by ambient temperature, amount of perspiration, exposure to water, abrasive removal, and other factors.

In general, higher concentrations of active ingredient provide longer duration of protection, regardless of the active ingredient, although concentrations above ~50% do not offer a marked increase in protection time. Products with <10% active ingredient may offer only limited protection, often from 1-2 hours. Products that offer sustained release or controlled release (micro-encapsulated) formulations, even with lower active ingredient concentrations, may provide longer protection times. Regardless of what product you use, if you start to get mosquito bites reapply the repellent according to the label instructions or remove yourself from the area with biting insects if possible.

These recommendations are for domestic use in the United States where EPA-registered products are readily available. See CDC Travelers’ Health website for additional recommendations concerning protection from insects when traveling outside the United States.

Repellents for use on clothing:
Certain products containing permethrin are recommended for use on clothing, shoes, bed nets, and camping gear, and are registered with EPA for this use. Permethrin is highly effective as an insecticide and as a repellent. Permethrin-treated clothing repels and kills ticks, mosquitoes, and other arthropods and retains this effect after repeated laundering. The permethrin insecticide should be reapplied following the label instructions. Some commercial products are available pretreated with permethrin.

EPA recommends the following precautions when using insect repellents:

  • Apply repellents only to exposed skin and/or clothing (as directed on the product label.) Do not use repellents under clothing.
  • Never use repellents over cuts, wounds or irritated skin.
  • Do not apply to eyes or mouth, and apply sparingly around ears. When using sprays, do not spray directly on face—spray on hands first and then apply to face.
  • Do not allow children to handle the product. When using on children, apply to your own hands first and then put it on the child. You may not want to apply to children’s hands.
  • Use just enough repellent to cover exposed skin and/or clothing. Heavy application and saturation are generally unnecessary for effectiveness. If biting insects do not respond to a thin film of repellent, then apply a bit more.
  • After returning indoors, wash treated skin with soap and water or bathe. This is particularly important when repellents are used repeatedly in a day or on consecutive days. Also, wash treated clothing before wearing it again. (This precaution may vary with different repellents—check the product label.)
  • If you or your child get a rash or other bad reaction from an insect repellent, stop using the repellent, wash the repellent off with mild soap and water, and call a local poison control center for further guidance. If you go to a doctor because of the repellent, take the repellent with you to show the doctor.

Note that the label for products containing oil of lemon eucalyptus specifies that they should not to be used on children under the age of three years. Other than those listed above, EPA does not recommend any additional precautions for using registered repellents on children or on pregnant or lactating women,. DEET-based repellents applied according to label instructions may be used along with a separate sunscreen. No data are available at this time regarding the use of other active repellent ingredients in combination with a sunscreen.
